Safety Alerts for Operators

Mitigating the Risks of a Runway Overrun Upon Landing - September 17, 2014, This SAFO provides ways for pilots and airplane operators to identify, understand, and mitigate risks associated with runway overruns during the landing phase of flight. It also provides operators with detailed information that operators may use to develop company standard operating procedures (SOP) to mitigate those risks.

Temporary Loss of Heading and Attitude Display on Garmin-Equipped Installations - August 21, 2014, This SAFO alerts owners, operators, repair stations, and mechanics to the temporary loss of displayed heading and attitude in aircraft equipped with the Garmin GRS 77/77H AHRS (Attitude Heading Reference System).

Global Positioning System (GPS)/Global Navigation Satellite System (GNSS) - March 10, 2014, This SAFO informs operators and maintenance personnel about the continuing reports of GPS/GNSS installations or upgrades in which integration with existing autopilots has resulted in less than ideal performance in some modes or conditions.

Cessna Citation CE-500 Type Aircraft Aileron Trim Systems - February 4, 2014, This SAFO serves to inform Cessna Citation CE-500 operators of sensitive aileron trim controls.

Re-categorization (RECAT) of FAA Wake Turbulence Separation Categories at Specific Airports - October 22, 2013, This SAFO has been updated with information regarding the expansion of RECAT of FAA Wake Turbulence Separation Categories at specific facilities.

Part 121 Air Carriers Performing Special Cargo Loads Operations - August 20, 2013, This SAFO serves to recommend tie-down procedures for restraint of special cargo loads.

Using Runways as Taxiways - August 13, 2012, This SAFO provides information on reducing the risk of runway incursions when taxiing on intersecting or active runways.

Title 14 of the Code of Federal Regulations (14 CFR) Part 121 Air Carriers Transporting Heavy Vehicle Special Cargo Loads - May 17, 2013, This SAFO advises operators of the potential safety impact of carrying and restraining heavy vehicle special cargo loads. The purpose is to reemphasize current policy and guidance concerning: weight and balance control procedures, cargo loading procedures, loading schedules and loading instructions.

Large Height Deviations in Oceanic Airspace - February 19, 2013, This SAFO advises operators of the potential impact on safety due to LHDs (300 ft or more from the cleared flight level) in oceanic airspace. Additionally, this SAFO directs operators to the International Civil Aviation Organization (ICAO) Oceanic Errors Safety Bulletin (OESB), which discusses current focus areas, standard operating procedures (SOP), and best practices. The OESB also recommends safety controls and mitigations, which should be incorporated into the operator’s international training programs.

Manual Flight Operations - January 4, 2013, This SAFO encourages operators to promote manual flight operations when appropriate.

Aircraft Approach Category - September 20, 2012, This SAFO serves to ensure aircraft operators are aware that the aicraft approach category used during an instrument approach is determined by the aircraft's Vref at the maximum certificated landing weight or 1.3 Vso at the maximum certificated landing weight.

Standard Operating Procedures (SOP) for Title 14 of the Code of Federal Regulations (14 CFR) Part 135 Certificate Holders and Part 91K Program Managers - July 9, 2012, The SAFO emphasizes the use of Standard Operating Procedures during all phases of flight; both normal and emergency. The document references a 2008 crash involving a Hawker 800 during a go-around attempt that killed two pilots and six passengers. An investigation suggested that the pilots deviated from SOPs following an aborted landing.  The SAFO not only recommends the use of SOPs by flight crews during all phases of flight, but calls on the certificate holders to establish a culture that promotes SOPs. The FAA recommends Advisory Circular (AC) 120-71A to develop, implement and update SOPs.

Safety During Positioning Flights - July 3, 2012, The Turbine Aircraft Operations Subgroup (TAOS), part of the General Aviation Joint Steering Committee, developed this SAFO to increase awareness in the aviation community about the frequency of accidents during positioning flights. This SAFO calls attention to the importance of adherence to established operating procedures and recognition of airplane limitations when conducting positioning flights.
